Scope and Purpose of Position Enhance care for Medicaid members with ongoing health‑related social needs (HRSNs). Serve as the primary contact throughout the social care journey, including screening, eligibility assessment, referral coordination, direct assistance, and follow‑up.

The role is community and office‑based, requires collaboration within and outside the Salvation Army, and involves significant interface with the client database. Expect up to 25% travel to other Salvation Army locations for on‑site service.

Responsibilities

Provide case/care management to Medicaid members, including screening, assessment, social care planning, and direct service delivery for HRSNs.

Manage incoming referrals for enhanced HRSN services within required timeframes.

Assess clients for Level 1 and Level 2 services; conduct and document outreach per required frequency and modality.

Maintain caseload levels to sustain position through SCN billable activities within 3 months.

Conduct HRSN screening using the Accountable Health Communities (AHC) tool; manage member consent and attestation.

Perform eligibility assessments and refer members to eligible programs, including Level 2 enhanced HRSN services and Level 1 federal, state, and local resources.

Develop and update social care plans detailing member needs, eligibility, and services.

Document progress notes and actions for each referral and service provision; ensure accurate data entry in the SCN client database.

Operate out of the primary location, providing support to Salvation Army centers across the SCN footprint; minimal travel to other sites as needed.

Engage Corps leadership, case management staff, and food program staff as required.

Communicate regularly with Divisional Headquarters Social Services team to discuss program progress, resolve issues, share feedback, and enhance member experience.

Complete all initial and ongoing SCN trainings and participate in annual professional development training.

Represent The Salvation Army at SCN and community meetings; attend staff meetings.

Adhere to Salvation Army policies and procedures.
