Role Overview We are looking for a hospice liaison who is committed to serving our patients with compassion and integrity. The liaison will coordinate patient referrals, establish relationships with local hospitals to secure referrals, and provide education for the community and referral sources on hospice care.
Responsibilities
Coordinate patient referrals and related documentation.
Develop and maintain relationships with local hospitals, medical community, and other referral sources to secure referrals.
Educate community members and healthcare professionals about hospice services.
Ensure continuity of care, proper communication, and correct documentation between the agency, hospitals, physicians, patients, and families.
Generate new hospice business in existing and new accounts; identify and pursue new market opportunities.
Coordinate care for referred patients and work closely with agency personnel to ensure efficient and effective patient care.
Perform duties regularly away from the employee’s place of business as required.
Qualifications
Heart to serve patients and families and a passion for providing the best possible care.
Education:
Minimum of an Associate Degree in a health-related field or proven work experience in a related area.
Experience:
2 years of sales experience in a clinical care setting, hospice preferred.
Required:
Reliable transportation; ability to sit, stand, bend, move intermittently; lift at least 25 lbs and bear the weight of an average adult effectively.
