Job Title: LifeSet Specialist
Program: LifeSet
Location: 591 Lakehurst Road, Toms River, NJ (Main Office)
Position Type: Full Time
Salary: $56,666.89
Job Description/Summary Provides individual services to 8-10 young adults in the home and community setting to assist the young adult with transition to adulthood, meeting a minimum of one time per week face to face with the young person and providing additional support via phone, text or in person as needed. When indicated, a Specialist may see a young person daily to ensure safety and stability. The small caseload allows the Specialist to be more intensive and intentional in the work with young people. The work schedule is flexible to meet the needs of the program and youth served and requires independence, and a high amount of travel across four service counties- Ocean, Monmouth, Burlington, and Camden.
LifeSet is an individualized, evidence-based community-based program that is highly intensive. LifeSet specialists meet with participants face to face at least once each week. They text, email, and call young people regularly throughout the week when needed. Specialists stabilize even the toughest situations and help young people build healthy relationships, obtain safe housing, education and employment. LifeSet is one of the nation’s first — and now one of the largest — evidence-based programs helping young people who age out of foster care.
Responsibilities
Demonstrates ability to practice job requirements in an accurate, timely basis and on a regular basis by: (1) Producing required work assignments in an accurate and prompt manner. (2) Performing job responsibilities in a professional manner. (3) Observing appropriate boundaries with clients and maintaining professional conduct with clients, co-workers, and supervisor(s). (4) Seeking supervision in an appropriate and professional manner. (5) Attending all scheduled meetings as required. (6) Meeting relevant continuing education requirements.
Responsible for all interventions associated with a specific young adult and any additional resources or supports. This includes case management and collaboration with external key stakeholders and referral sources, such as probation, child welfare workers, therapists etc.
Demonstrates ability to facilitate development of an Individual Treatment Plan by partnering with the youth that are strength based, solution based and are person centered, including specifying future objectives, evaluating the means for achieving them, identifying appropriate community resources and key stakeholders, and making deliberate choices about appropriate courses of action in order to enhance the youth’s social well-being, independence and behavioral wellness.
Participates in an on-call rotation and is expected to respond in person at any time when it is determined to be necessary by the Supervisor, LifeSet Model Expert and Program Director.
Attends weekly individual development, clinical supervision and consultation meetings lead by a licensed professional who is an expert in the LifeSet model.
To adhere to all ethical and legal guidelines, including confidentiality and mandated reporting. Specialists are a part of a national network across multiple public and private organizations all striving to enhance the standard of care for older youth.
Attends all team meetings; attends mandatory orientation and trainings; demonstrates ability to perform basic computer functions as required by job function; performs job responsibility with a minimum of supervision from Directors; follows policies and procedures outlined in Employee Handbook; attends at least two agency held quarterly meetings per year; demonstrates familiarity with organization and program specific policies; all other duties as assigned.

Practices a trauma informed philosophy as demonstrated through behaviors and communications, embracing it as a cultural value.
Demonstrates ability to maintain confidentiality in the daily operations of the department/agency; immediately reports to management breaches of confidentiality.
Demonstrates ability to assist in de-escalating heightened situations that do not require emergency intervention (screening or police)
Demonstrates ability to effectively communicate orally and in writing; completes all required documentation accurately and within the established timeframes.
Qualifications
Education- Master’s degree in field of social work, psychology or related field preferred or bachelor’s degree in social services with one-year related experience in counseling and or case management or other related experience is required.
Experience with one-year related experience in counseling and or case management or other related experience is required.
Candidates should also possess strong organizational skills and attention to detail, the ability to maintain a flexible schedule, the ability to manage multiple priorities simultaneously, and excellent written and verbal skills.
Bilingual in Spanish for additional compensation
Valid NJ Driver’s License
Willingness to adapt to high level of travel amongst 4 counties - Ocean, Monmouth, Burlington, and Camden.
