The Social Worker (MSW) is responsible for performing patient care activities, including assessments, contributing to care plans, discharge planning, and providing education and support to patients, families, and caregivers. Adheres to the NASW Code of Ethics. Responsibilities
Assess medical, cognitive, psychosocial, and financial status of patients and families. Identify patient needs, strengths, barriers, and resources. Develop care and discharge plans collaboratively. Provide education and support to patients, families, and caregivers. Coordinate referrals and linkages to community resources. Maintain clinical documentation and communicate with the care team. Build relationships with community providers and participate in outreach. Prioritize caseloads based on needs and resources. Minimum Requirements
Master of Social Work (MSW). Active driver’s license and transportation access. Cultural competency and experience with diverse backgrounds. Current BLS certification. Job Details
