Description
Conduct assessments for patients in inpatient or outpatient settings and create tailored treatment plans
Implement physical therapy interventions emphasizing mobility, strength, balance, and function
Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
Inform patients and their families about therapeutic aims, discharge planning, and at‑home exercise routines
Maintain accurate documentation of evaluations, treatments, and patient progress in accordance with facility and regulatory standards
Engage in care team discussions and interdisciplinary meetings to ensure cohesive treatment plans
Education
Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution
Certifications
State licensure as a
Physical Therapist
CPR certification preferred
Specialized certifications (such as neurological or geriatric) are beneficial but not required
Skills
Previous experience strongly preferred
Capability to treat patients with complex medical and mobility needs
Excellent communication and collaboration skills
Proficient in using digital charting and clinical documentation software
