Director of Case Management
for our client in Nashville TN.
The Director of Case Management is responsible for the strategic leadership, operational oversight, and day-to-day management of the hospital’s case management department. This role ensures effective care coordination, discharge planning, regulatory compliance, and optimal use of clinical resources across the continuum of care. The Director partners closely with interdisciplinary teams and external stakeholders to support quality outcomes, patient flow, and performance goals. This leader serves as a subject matter expert in hospital case management and is accountable for departmental results through strong leadership, collaboration, and continuous improvement. This position requires a confident, results-driven professional with strong executive presence, excellent communication skills, and the ability to lead through change in a fast-paced healthcare environment.
Key Responsibilities Provide leadership and oversight of all case management operations, including staffing models, workflows, onboarding, competencies, and use of technology Ensure appropriate resource allocation to meet productivity, quality, and patient care goals Maintain oversight of core case management functions, care coordination, and discharge planning practices Ensure timely, high-quality documentation and compliance with regulatory and organizational standards Collaborate with finance and senior leadership to support departmental performance and operational goals Analyze operational data and performance metrics to identify opportunities for improvement and implement sustainable solutions Build strong working relationships with clinical leaders, ancillary departments, and organizational partners Promote a culture of accountability, collaboration, and continuous improvement Uphold organizational values, ethical standards, and professional conduct expectations Perform additional duties as needed to support departmental and organizational objectives Qualifications Bachelor’s degree required Master’s degree in Nursing, Health Administration, or Business Administration preferred Active licensure as one of the following: Registered Nurse (RN) Licensed Clinical Social Worker (LCSW) Licensed Master Social Worker (LMSW) Minimum of 3 years of recent experience in acute care hospital case management At least 2 years of leadership or management experience within case management or care coordination If interested, apply now!
