Position Summary: As an independent lease holder, you will provide eye and vision care within Walmart stores, leading patient care and ensuring quality service. You will control your schedule, office hours, fee schedule, and keep 100% of exam/professional fees.

Minimum Qualifications
- Doctor of Optometry (OD) from an accredited optometry school.
- Valid state optometry license.
- Ophthalmologist (MD or DO) from an accredited school.
- Valid state ophthalmology license.
- General Liability Certificate.
- Professional Liability Insurance Certificate.
- W‑9.
