Position Summary: As an independent lease holder in Walmart stores, you'll launch your services and take a leading role in patient care for eye, vision, and general health. You will establish your schedule, hours, and fee structure, and keep 100% of exam and professional fees.

Optional Options for a fair market value
- Equipment/Instrumentation
- Administrative and/or Opt-Tech support services (if state allows)
Minimum Qualifications
- Doctor of Optometry (OD) from an accredited optometry school and valid state optometry license
- Ophthalmologist (MD or DO) from an accredited school, valid state ophthalmology license, General Liability Certificate, Professional Liability Insurance Certificate, and W-9
ME & VA license agreements are with Gumberg Asset Management Company.
