Evaluate patients in inpatient or outpatient care and design tailored treatment plans to meet individual needs. Implement physical therapy interventions to enhance mobility, strength, balance, and overall function. Collaborate with nurses, physicians, case managers, and other rehab specialists to optimize patient outcomes. Educate patients and families on therapeutic goals, discharge planning, and essential home exercises. Document evaluations, treatments, and progress in accordance with facility and regulatory guidelines. Participate in interdisciplinary rounds and care team conferences to coordinate patient care. Assist with discharge planning to ensure safe transitions home. Qualifications
Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution. State licensure as a Physical Therapist; CPR certification preferred. Specialized certifications in geriatric or neurological care are a plus but not required. Previous experience in addressing complex medical and mobility issues is valued. Strong communication skills and ability to work effectively within a multidisciplinary team. Proficiency in digital charting and clinical documentation systems. Compensation & Benefits
