Description

Assess patients in both inpatient and outpatient environments and create customized treatment plans tailored to individual needs

Deliver physical therapy interventions that enhance mobility, strength, balance, and functional performance

Work alongside nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes

Guide patients and their families through therapeutic objectives, discharge planning, and home exercise protocols

Maintain thorough documentation of evaluations, treatments, and patient progress according to established facility and regulatory standards

Contribute to collaborative care rounds and interdisciplinary discussions to ensure cohesive patient care

Education

Doctorate in Physical Therapy (DPT) or equivalent degree from a recognized institution

Certifications

State licensure as a

Physical Therapist

CPR certification is preferred

Any specialized certifications (such as geriatrics or neurology) are advantageous but not mandatory

Skills

Prior experience strongly preferred

Capability to address the needs of patients with multifaceted medical and mobility concerns

Excellent communication and teamwork abilities

Proficiency in using electronic health records and clinical documentation systems
