Principal Responsibilities

Plan, organize, implement, and evaluate a comprehensive Social Services program

Develop and maintain social services policies related to resident care and quality of life

Prepare the Social Services department for annual surveys and inspections

Collaborate with the interdisciplinary team to promote resident rights and psychological well-being

Prevent, identify, and report resident abuse, neglect, or exploitation in accordance with law and licensure

Complete psychosocial assessments, social histories, and applicable MDS sections to guide care planning

Maintain accurate and timely documentation in compliance with federal, state, and company requirements

Participate in the development and implementation of interdisciplinary care plans

Provide therapeutic interventions to assist residents with transitions and adjustment to long-term care

Educate residents and families on advance directives, resident rights, and available community resources

Coordinate discharge planning, including referrals for home care and post-discharge services

Initiate and participate in Discharge Transition Plans and discharge education

Maintain relationships with community agencies and referral partners

Participate in quality assurance and interdisciplinary team meetings

Educate staff on psychosocial needs, cultural diversity, resident rights, and abuse prevention

Collaborate with nursing leadership and administration during inspections and surveys

Provide clinical support during crisis or catastrophic events

Perform other duties as assigned to support resident well-being and facility operations

Qualifications

Associate degree in a human services field or related social services experience (Bachelor’s degree preferred)

Minimum of

one year of experience

in a human services role within a hospital, skilled nursing, or medical facility

Willingness to obtain

Social Services Designee certification

if not already obtained

Knowledge of resident rights, psychosocial needs, and long-term care regulations

Strong communication, organization, and documentation skills
