Clinical Liaison / Marketer Key Responsibilities
Build and maintain strong referral relationships with hospitals, case managers, physicians, and discharge planners.
Screen and evaluate potential admissions for clinical appropriateness.
Coordinate admissions with internal interdisciplinary teams.
Educate referral sources on SNF services, outcomes, and care capabilities.
Track and follow up on referrals to support census and occupancy goals.
Ensure compliance with regulatory, clinical, and documentation standards.
Clinical Liaison / Marketer Qualifications
Experience in skilled nursing, acute care, or post-acute care preferred.
Strong understanding of discharge planning and care transitions.
Excellent communication, organization, and relationship-building skills.
Marketing, liaison, or admissions experience a plus.
Ability to work independently and manage multiple priorities.
