Position Overview We are seeking a motivated, sales‑driven Homecare Sales Representative to join our Brooklyn‑based team. This is a relationship‑focused role centered on guiding patients and families from their very first call through successful enrollment into homecare services.

This position is ideal for someone who is confident, compassionate, organized, and comfortable closing enrollments. You will act as the primary point of contact for prospective patients, ensuring a smooth and supportive onboarding experience while meeting performance and enrollment goals.

Key Responsibilities Patient Enrollment & Sales Conversion

Handle patient enrollment from the initial inquiry call through assessment scheduling

Educate patients and families about available homecare services and eligibility requirements

Identify patient needs and clearly explain next steps in the enrollment process

Confidently guide families toward completing the enrollment process

Follow up consistently to ensure conversions are completed in a timely manner

Medicaid Eligibility & Authorization Support

Conduct Medicaid eligibility checks and verify coverage

Explain benefits and program requirements to families

Guide patients through necessary documentation and approval processes

Follow up to secure timely authorizations and ensure smooth onboarding

Communicate with insurance representatives as needed

Communication & Coordination

Serve as the communication hub between marketers, patients, caregivers, and internal office teams

Coordinate scheduling of nursing assessments and intake appointments

Ensure all stakeholders are informed and aligned throughout the enrollment process

Maintain professional and compassionate communication at all times

Documentation & Compliance

Manage and track all required enrollment documentation

Ensure forms are collected accurately and completely

Maintain organized records in compliance with company and regulatory requirements

Monitor progress to prevent delays in services startingli>
