Job Title Community Outreach Physician Liaison
Location Westminster, CO
Compensation $68‑72k/year
Position Overview The Community Outreach Physician Liaison integrates patient engagement, community outreach, and physician relationship management to enhance patient retention, increase intakes, and strengthen Giving Home Health Care’s (GHHC) regional presence. This role combines proactive patient and caregiver support with strategic physician collaboration to reduce transfers out, grow referral networks, and ensure seamless care transitions. The Community Outreach Physician Liaison serves as a bridge between patients, community stakeholders, and medical providers, driving organizational goals through personalized outreach, physician education, and data‑driven performance.
Qualifications
Bachelor’s degree in Healthcare, Sales, or a related field preferred
Minimum 3 years of experience in home health care, outreach, community engagement, or physician liaison roles
Proven success in relationship‑building, patient retention, and referral generation
Strong communication, organizational, and problem‑solving skills
Proficiency in Salesforce or similar CRM systems required
Responsibilities
Patient Engagement and Retention
Conduct face‑to‑face home visits and phone calls to build trust, address concerns, and reinforce GHHC’s value
Ensure 100% of patients receive monthly/quarterly touchpointsIdentify at‑risk patients via surveys, caregiver feedback, and proactive outreach
Utilize CQ/HME letters during visits to negotiate pay rates and prevent transfers
Implement retention plans with structured follow‑ups
Community Outreach and Growth
Develop and execute outreach strategies to boost brand awareness and patient acquisition
Build relationships with local unions, community organizations, and stakeholders to generate referral sources
Physician Relationship Management and Support
Contact patients after intake to schedule physician appointments with supportive PCPs or specialists
Present causation packets (letters, medical records, cited studies, procedure manual) and educate providers on DOL requirements and response types
Confirm appointments and upload signed documents to Salesforce
Physician Onboarding and Referral Growth
Onboard new supportive MDs by attending initial appointments, explaining GHHC processes, and establishing communication preferences
Obtain new patient referrals from physician/clinic relationships
Compensation & Perks
Base Salary: $68‑72k/year

$100/month cell phone reimbursement
