Key Responsibilities
Resident & Family Support
Guide residents and families through the admissions process, providing education, reassurance, and emotional support
Meet with new residents to assist with adjustment, address concerns, and provide counseling as needed
Educate residents and families on resident rights, responsibilities, and available services
Build trusting, meaningful relationships with residents and families, serving as a consistent point of support
Assessment & Care Planning
Complete social histories, psychosocial assessments, and required documentation, including the MDS social services section
Participate in interdisciplinary care planning to ensure resident goals, preferences, and needs are reflected in care plans
Monitor resident well-being and advocate for care plan updates as needs change
Advocacy & Transitions of Care
Support residents during room changes, care transitions, and life adjustments with compassion and follow-up
Identify changes in resident behavior or emotional status and communicate concerns promptly to the care team
Serve as discharge coordinator, developing safe, realistic discharge plans in partnership with residents, families, and community agencies
Community Connection & Resources
Connect residents and families with community resources and services that support continued well-being
Establish and maintain positive relationships with community agencies and referral partners
Represent the facility with warmth and professionalism to community groups and referral sources
Compliance & Resident Rights
Advocate for resident rights and investigate concerns or allegations, reporting findings appropriately
Coordinate legal status changes such as guardianships and protective placements
Ensure services are delivered in compliance with applicable regulations and standards

Qualifications:
Bachelor's or Master's in Social Work or related field
Experience in social services, behavioral health, or long-term care
Strong communication, leadership, and relationship-building skills
