Fairview Skilled Rehab & Nursing Home is seeking an experienced Director of Social Work to lead and oversee the Social Services Department. The Director will supervise social work staff, support residents and families, and ensure compliance with all state and federal regulations.
Responsibilities
Oversee daily operations of the Social Services Department
Supervise and support social workers and social service staff
Complete psychosocial assessments and care plans for residents
Participate in interdisciplinary care plan meetings
Provide counseling and support to residents and families
Coordinate discharge planning and community resources
Address resident and family concerns
Maintain accurate documentation in the EMR
Ensure compliance with NYS Department of Health and CMS regulations
Advocate for residents’ rights and overall well-being
Qualifications
LCSW or LMSW required (New York State license)
Strong communication and organizational skills
Knowledge of long‑term care regulations and discharge planning
Experience In Skilled Nursing Or Rehabilitation Preferred
Leadership or supervisory experience preferred
