Duties and Responsibilities: Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs. Demonstrates knowledge, skills, and techniques necessary to plan social services programs for residents with the following needs: therapeutic, neurological and dementia, dialysis, IV therapy, and infectious diseases if applicable. Promptly follows-up on admission inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement

Qualifications: Experience as a Director of Social Work in a large LTC setting preferred BSW or MSW preferred, or Bachelors degree in related field At least one year experience in Social Work field required
