Provide emotional and social support to residents and families Oversee admissions, discharge planning, and care transitions Advocate for residents rights and well-being Ensure compliance with state and federal regulations Work closely with interdisciplinary teams to enhance resident care
Requirements:
Experience in social services in a long-term care setting Knowledge of Medicare, Medicaid, and discharge planning Strong communication and problem-solving skills Bachelor's degree in Social Work or related field (preferred)
