Occupational Title: Director of Nursing (DON) Department: Home Health and Hospice Reports To: Administrator Facility Location: Blue Mountain Hospital Cedar Mesa Position: Full-time Authority: The Director of Nursing has the authority to implement and enforce clinical policies, supervise and evaluate clinical staff, recommend disciplinary actions, remove staff from patient care if safety is compromised and recommend staffing adjustments to meet regulatory requirements. The Director of Nursing (DON) is responsible for the overall direction, supervision, and coordination of clinical services for both the Home Health and Hospice programs. The DON ensures that care is delivered safely, effectively, and in compliance with CMS Conditions of Participation (42 CFR Parts 484 and 418), Utah state regulations, and agency policies as well as joint commission accreditation standards. The DON ensures 24-hour nursing availability, oversees interdisciplinary coordination, leads Quality Assurance and Performance Improvement (QAPI) activities and maintains continuous survey readiness. Key Performance Indicators
Overall Clinical Oversight (Both Programs):
Direct and supervise all clinical services; Ensure care is delivered in accordance with physician orders and individualized plans of care; Oversee clinical documentation compliance; Ensure patient safety and regulatory adherence; Maintain clear lines of clinical authority Home Health-Specific Oversight (42 CFR Part 484):
Ensure appropriate supervision of home health clinical staff; Oversee OASIS accuracy and timely submission; Ensure home health aide supervisory visits occur per regulation; Ensure timely initiation of care; Monitor hospitalization and outcome data Hospice-Specific Oversight (42 CFR Part 418):
Ensure nursing services are available 24 hours per day, 7 days a week; Oversee symptom management and crisis response processes; Ensure RN supervisory visits for hospice aides occur at least every 14 days; Participate in and oversee the Interdisciplinary Group (IDG); Ensure comprehensive assessment updates at least every 15 days Quality Assurance and Performance Improvement (QAPI):
Lead QAPI programs for both Home Health and Hospice; Monitor Quality Indicators such as Hospital readmissions, Pain and symptom management effectiveness, Infection rates, Falls; Analyze trends and implement corrective actions; Prepare reports for governing body review Staffing and Supervision:
Participate in hiring and orientation of clinical staff; Ensure competency validation at hire and annually; Conduct performance evaluations; Ensure adequate staffing levels for both programs; Provide clinical coaching and mentorship Regulatory Compliance & Survey Readiness:
Maintain ongoing compliance with CMS and Utah regulations; Participate in state and federal surveys; Develop and implement plans of correction as needed; Ensure policies and procedures are current and implemented After-Hours & Emergency Oversight:
Ensure effective 24/7 on-call systems; Monitor response times for both programs; Oversee documentation of after-hours calls; Participate in emergency preparedness planning and annual review Infection Prevention & Control:
Oversee infection control programs for both services; Monitor trends and outbreaks; Ensure staff training and compliance Working Conditions:
Combination of office and field responsibilities; Travel to patient homes; Participation in on-call rotation as needed Qualifications & Experience
Current, active Registered Nurse (RN) licensure in Utah Bachelor’s degree in Nursing preferred Two (2) years RN experience One (1) year supervisory/management experience Home health and/or hospice experience Current CPR certification Valid driver’s license and reliable transportation Current BLS Certification Knowledge of CMS Home Health and Hospice Conditions of Participation (CoPs) Minimum typing speed: 30 words per minute Knowledge of EMR systems Competent in Google Systems (Sheets, Calendar, Documents, Slides) General Requirements
In addition to the specific duties listed above, the Employee is required to do the following: tasks, projects, or actions that are reasonably within the scope of the Employee’s position or employment, which the Employee’s supervisor, any member of BMH’s management, or BMH’s administration team—asks of the Employee. Tasks, projects, or actions are “reasonably within the scope” of the Employee’s employment unless they are so far removed therefrom that no legitimate argument can be made that such tasks relate to BMH or the Employee’s employment. Act professionally in a medical organization, respecting licensed medical providers and behavioral health providers. Keep all BMH information confidential and adhere to confidentiality policies and laws including HIPAA. Avoid conduct that could cause concern or annoyance to any BMH patient toward BMH as an organization or employee. Work congenially and cooperatively with all BMH employees; maintain a pleasant demeanor and comply with management requests in a timely manner; refrain from gossip about employees or patients. Communicate complaints about BMH through the appropriate management channels; escalate as needed within the chain of command. Check the Employee’s BMH email as applicable. Do not disclose Private BMH Information to media or non-BMH entities without formal approval from the CEO; defined as confidential policies, HIPAA, internal business information, and information about employees.
