As an Enrollment Specialist, you’ll focus on identifying and engaging eligible individuals and guiding them through enrollment into CareCollab’s Health Home and Social Care Network (SCN) programs. You’ll spend much of your time in the field and on the phone—connecting with people, explaining program benefits, verifying eligibility, and completing accurate enrollments.

This role requires persistence, strong communication skills, comfort with outreach goals, and the ability to follow through in fast-paced, community-based environments.

What You’ll Do

Conduct high-volume outreach through phone calls, in-person visits, and community events

Engage eligible individuals and clearly explain Health Home and SCN program benefits

Verify eligibility, obtain signed consents, and complete enrollment documentation

Enter and maintain accurate records in JotForm and Foothold Care Management (FCM)

Collaborate with Intake and Enrollment teams to manage referrals and meet enrollment targets

Participate in community engagement activities to increase program visibility and referrals

Track outreach activity and follow up consistently with prospective members

What You Bring

Minimum of 1 year of experience in outreach, enrollment, sales, canvassing, or community engagement

Experience working with Medicaid populations or vulnerable communities is a plus

Strong verbal communication skills and comfort initiating conversations with new people

Ability to handle rejection and remain persistent in outreach efforts

Organized, reliable follow-through, and strong time management

Comfortable using EMRs, CRMs, and digital documentation tools

Ability to work independently in the field and in office-based settings

Bilingual skills preferred

Familiarity with NYC communities is a plus
