We are seeking an Integrated Health Care Navigator; as an IHCN, you'll manage our
virtual PrEP care program (Mistr) , provide
HIV/STI counseling and testing , and guide clients in accessing critical health services—ensuring every person you serve has access to inclusive, affirming, and lifesaving care.
Location : Chicago, IL
Key Responsibilities:
Direct Client Services
Conduct HIV/STI walk‑in testing and risk‑reduction counseling.
Link clients to PrEP, HIV care, and supportive services.
Collaborate with trusted partners: Howard Brown, UChicago, Mt. Sinai, and more.
Program & Data Management
Track client enrollment, testing outcomes, and referrals.
Submit accurate reports and ensure compliance with program goals.
Outreach & Engagement
Support retention and awareness for virtual PrEP programming.
Represent Project Vida in the community, building partnerships.
Professional Development
Attend conferences, trainings, and maintain certifications (HIV, STI, Phlebotomy).
Contribute to fundraising and advocacy efforts for health equity.
Qualifications and skills:
HIV Counseling/Testing Certification (Required).
2+ years of community health, HIV prevention, or public health experience.
Excellent communication, documentation, and outreach skills.
Culturally competent and passionate about serving marginalized communities.
Able to work flexible hours, including evenings and Saturdays.
Reliable transportation and tech‑savvy (MS Office proficient).
