A Day in the Life The RN, Director of Case Management will be responsible for overseeing and advancing all case management functions within our organization. This role is pivotal in ensuring clients receive comprehensive, quality services while driving efficiency, compliance, and innovation within our case management processes. The ideal candidate will have a strong background in case management leadership, client advocacy, and regulatory compliance.

How This Position Contributes to Our Mission

Develop and implement case management strategies to ensure timely, high-quality service delivery to clients.

Lead and mentor the case management team to build a culture of accountability, compassion, and excellence.

Ensure all case management practices comply with organizational policies, funding requirements, and federal/state regulations.

Collaborate with interdisciplinary teams to develop and refine care plans that meet the diverse needs of clients.

Monitor and analyze case management metrics, providing insights and recommendations for continuous improvement.

Serve as a resource for escalated client concerns, ensuring resolutions align with organizational values and objectives.

Oversee documentation and reporting processes to ensure accuracy, timeliness, and compliance.

Drive innovation in case management, implementing tools, technology, and strategies to improve service delivery and outcomes.

Skills and Qualifications

Active and unencumbered KY registered nurse license.

Minimum of a bachelor’s degree in social work, nursing, healthcare management, or a related field. Master’s degree preferred.

At least five years of progressive leadership experience in case management, preferably in healthcare, social services, or nonprofit settings.

Proven track record of managing complex caseloads and exceeding performance targets.

Deep understanding of regulatory requirements, including HIPAA, Medicaid, and other relevant standards.

Exceptional interpersonal and communication skills with the ability to build trust and collaboration across teams and stakeholders.

Demonstrated ability to drive results in a fast-paced, mission-driven environment.

Strong organizational skills with an emphasis on detail, problem-solving, and multitasking.

Proficiency in case management software and tools.

Certification in Case Management (CCM or equivalent) is a plus.
