Social Services Director

Role This is a full-time, on-site Social Services Director role based in Du Bois, PA. The Social Services Director will oversee and coordinate social service programs, manage care planning and discharge planning, provide case management, and support residents' social well-being. They will collaborate with staff, residents, and families to ensure individualized care plans are executed effectively and meet the diverse needs of the community. The Director will also serve as a key liaison with other healthcare professionals and community resources. Qualifications Proficiency in Social Services, Social Work, and expertise in supporting individuals and families Experience creating and managing tailored Care Plans to address individual needs Strong skills in Discharge Planning, ensuring smooth transitions and support for residents Expertise in Case Management, ensuring personalized and effective service delivery Excellent interpersonal and communication skills for collaborating with residents, families, staff, and external partners Bachelor's or Master's degree (MSW preferred ) in Social Work or a related field Previous experience in a healthcare or long-term care setting is strongly preferred Working knowledge of state and federal regulations related to social services in healthcare facilities
