About Us
Touching Hearts at Home serves Sarasota and Charlotte Counties with compassionate, relationship-based, non-medical in-home care. We help seniors and adults remain safe, independent, and connected at home—through companionship, personal care support, dementia/Alzheimer’s support, post-hospital support, and daily living assistance.
Position Overview
This is a hands‑on leadership role combining client services operations with community referral development. You’ll be the hub that keeps intake, onboarding, and scheduling moving—while building trusted relationships with local referral partners.
Responsibilities
Manage inbound leads: answer calls, qualify needs, schedule assessments, follow up
Coordinate client onboarding and start‑of‑care checklists
Coordinate caregiver onboarding readiness and documentation
Support scheduling and coverage (including call‑outs and urgent fill needs)
Maintain CRM/referral database and weekly activity reporting
Build referral relationships (SNF/ALF/rehab/hospitals/physicians/geriatric care managers/elder law)
Represent the agency at community events and partner meetings
Qualifications
2+ years in home care, hospice, home health, senior living, or healthcare outreach
Proven relationship-building and follow‑up discipline
Strong phone skills and comfort with intake/conversion
Organized, checklist‑driven, and calm under pressure
Valid driver’s license and reliable transportation (local travel)
Compensation & Benefits
Base salary: $50,000–$68,000 (DOE)
Performance bonus: per new client start + retention bonus
Mileage reimbursement for outreach
PTO + paid holidays (customize)
Growth opportunity as the agency scales
