The above reflects the anticipated hourly wage range for this position if hired to work in New Jersey. The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.
Job Overview The Outreach Community Health Worker will primarily maintain a presence in the community with patients to promote, motivate and improve healthy living. The CHW will assist patients in utilizing resources and accessing services to overcome barriers in order to meet their health goals.
Essential Functions
Meet patients in a clinic, hospital, ER, and where patients are located and conduct needs assessment using a Social Determinants of Health Tool
Schedule weekly follow‑up calls and home visits to patients
Help patients with social issues such as homelessness, substance abuse, hunger, transportation, and other social issues impacting their lives
Assist patients with organizing records, making follow‑up appointments with Primary Care Physicians or Specialists
Help patients with tasks of getting medical care, and work on health goals by planning healthy meals or finding time to exercise
Help patients access resources, including quality of care, childcare services, food, housing, and transportation
Maintain relationships with Community‑Based Organizations
Able to travel and meet the members where they are; may include local churches, homeless shelters, and members’ residences

Qualifications
High School Diploma or equivalent
Scheduling
Full time, Day shift. 9‑5 PM
Onsite daily at Newark Beth Israel Medical Center
