Physical Therapy Assistant (PTA) - Santa Cruz Post Acute Full-Time, Part-Time, and PRN opportunities are available.

Starting at $45/hour (DOE) + Benefits.

Position Summary Santa Cruz Post Acute is seeking a compassionate and skilled Physical Therapy Assistant (PTA) to join our high‑performing rehabilitation team. We provide quality, resident‑centered care in a supportive and collaborative skilled nursing environment. The Physical Therapy Assistant works under the direction and supervision of a licensed Physical Therapist to deliver effective therapeutic interventions aimed at improving resident mobility, strength, endurance, balance, and functional independence.

Responsibilities

Provide direct physical therapy treatments in accordance with individualized care plans established by the Physical Therapist.

Assist residents with therapeutic exercises, gait training, mobility programs, and functional activities.

Document daily treatment notes, resident progress, and response to therapy in accordance with facility and regulatory guidelines.

Collaborate with the PT, therapy team, nursing staff, and other interdisciplinary team members to support resident‑centered care goals.

Educate residents and caregivers on exercise programs, safety, and mobility techniques.

Maintain compliance with state/federal regulations and professional standards.

Qualifications

Current Physical Therapy Assistant (PTA) license in the state of California.

Graduate of an accredited PTA program.

Strong communication, teamwork, and documentation skills.

SNF experience preferred, but new graduates are welcome to apply!
