LPN Client Relationship Specialist (100% In Office)

Responsible for developing and maintaining client relations and intake of direct referrals made to HCR. Essential Functions Cultivated relationships to help our physicians and hospitals identify the best home care solutions for their patients. Demonstrated ability to work in fast-paced environment while maintaining attention to detail. Timely and accurate creation & processing of incoming referrals. Demonstrated effective communication with referral sources and other HCR departments. Arranges Durable Medical Equipment/Oxygen, etc. with outside vendors. Confirms insurance coverage on all referrals. Works in conjunction with Insurance Specialists to obtain required verifications/authorizations. Documents according to department, agency & regulatory guidelines. Actively participates in department and agency meetings with solutions-oriented contributions. Other duties as assigned. Education Requirements Completion of an approved program of practical nursing. Valid LPN license issued by NYS Dept of Education. Qualifications and Requirements Provider relations experience in a healthcare environment preferred. Exceptional customer service skills, with a good comfort level working in a fast paced office setting. Top knotch computer proficiency and efficiency (MS Office Suite, Electronic Medical Record system EMR) Possesses a keen eye for details, excellent time management abilities. Meets or exceeds established productivity expectations with the end goal of increasing productivity. The Client Relationship Specialist LPN is primarily in an office setting and may be exposed to outdoor conditions. The working conditions are classified as sedentary work: Sedentary work - Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.
