Social Work Director
A skilled nursing, long term care facility located in Suffolk County is seeking a strong Social Work Director.
Job Description of the Social Worker
Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living
Provide information to the resident, and/or family member, on residents' rights and responsibilities
Interview residents/families to obtain social history, and participate in discharge planning, development and implementation of social care plans and resident assessments. Ensure that all charted progress notes are informative and descriptive of the services provided and the resident’s response to those services.
Interpret social, psychological and emotional needs of the resident care team members, and involve the resident/facility in planning objectives and goals for the resident.
Refer resident/families to appropriate social services agencies as needed and arrange transportation to other facilities when necessary.
Qualifications/Requirements
Must have experience as a Social Worker in a Nursing Home
Strong written and verbal skills
Must have excellent communication skill
