We are currently seeking a Social Services Designee.

Job Responsibilities and Duties:

Plan, develop, organize, implement, evaluate, and direct the overall operations of the Social Services Department

Assist with facility admissions

Manage resident trust accounts and assist with resident shopping

Schedule resident appointments and coordinate with other departments as needed

Maintain accurate charts and documentation of resident care

Engage, converse, and interact with residents to promote a positive environment

Work primarily during daytime hours, with occasional evening and weekend visits as required

Requirements:

Prefer 2-5 years of Social Work experience, ideally in Long Term Care setting.

At least 1 year of supervised social work experience in health care setting working directly with individuals including those with mental challenges.

Knowledge about Medicaid and Medicare programs is a plus.
