We are working with a dynamic healthcare/social services organization that is seeking an Intake Specialist to perform intake, screening, and triage, helping connect individuals with appropriate programs and services. What You’ll Do
Provide information and access to quality services for older adults that help keep them at home and in their community
Conduct intake and screening of callers to determine needs and appropriate services
Assist in triage by prioritizing cases based on urgency and available resources
Maintain accurate records of client interactions and referral information
Perform additional duties as required to support departmental goals
What We’re Looking For
2+ years of experience in social work or human services
Bachelor’s degree is a MUST
Strong communication and interpersonal skills
Skilled in engaging with older adults and family members
Proficient in Microsoft Office (especially Word and Excel), with strong typing (45+ WPM)
Bilingual (English/Spanish)
Compassionate, detail‑oriented, and client‑focused approach
