Social Services Director position -

Social Worker with experience in

long-term care to support the social and emotional well-being of our residents.

Key

Responsibilities, but not limited to :

Support residents in maintaining or improving physical, emotional, and psychosocial well-being

Identify and assess individual social and emotional needs

Create and update personalized care plans

Participate in admission planning, discharge coordination, and transfers

Collaborate in interdisciplinary care plan meetings

Partner with finance to ensure residents understand documentation and financial needs

Coordinate discharge planning, including DME and home care services for safe community reintegration

Qualifications:

Minimum of a bachelor's degree in social work or a bachelor's degree in a human services field

Minimum

1 year of experience

in Social Work, in

Long-Term Care

Experience working with aging and diverse populations
