Summary:
The Patient Facilitated Services Director is responsible foroptimizingpatient access and coordination on behalf of Philanthropy across thesystem. This role ensures thatphilanthropy contacts have a single point of contact tofacilitatetimely, efficient, and compassionate support throughout their care journey. This role will lead program strategy, design operational workflows, manage schedulinglogistics, and serve as a key liaison between colleagues, community members, providers, administrativeteamsand executive leadership.
This position will be onsite and may include evenings and weekends as needed.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: Bachelors degree in healthcare administration, nursing, social work, or a related field. Masters preferred
- License/Certifications: Registered nurse, Licensed Clinical Social worker or related field.
- Experience: Minimum 6 years of experience in a clinical healthcare setting, working knowledge of healthcare operations, patient services, and care coordination in academic medical centers or large healthcare system.
