Job Summary
We are seeking an experienced Director of Case Management to lead and oversee hospital case management operations. This leadership role is responsible for ensuring effective care coordination, discharge planning, regulatory compliance, and optimal utilization of clinical resources. The ideal candidate will bring strong leadership, strategic thinking, and the ability to drive performance outcomes in a fast-paced acute care environment.
Key Responsibilities
Lead and manage all case management operations and staff Oversee care coordination and discharge planning processes Ensure compliance with organizational and regulatory standards Monitor performance metrics and implement process improvements Collaborate with interdisciplinary teams and leadership stakeholders Manage staffing, productivity, and departmental resources Drive quality outcomes and patient-centered care initiatives Qualifications
Bachelor's degree required (Master's preferred) Active RN, LCSW, or LMSW license required 3+ years of acute care case management experience 2+ years of leadership experience in case management Strong knowledge of hospital operations, compliance, and care coordination Benefits
