FACETS is seeking mission-driven Case Managers to join our team providing comprehensive, community-based case management services to individuals experiencing homelessness or housing instability. Case Managers help clients access essential services, secure stable housing, and work toward long-term stability and self-sufficiency. We address urgent needs through wraparound services and strong community partnerships, helping individuals and families move toward safe, stable housing after years of uncertainty and instability. Conduct comprehensive assessments to identify client needs and develop individualized service plans Connect clients to housing resources, healthcare services, and community-based supports Assist with housing applications, documentation, and housing stability planning Conduct field-based outreach to engage individuals experiencing homelessness, including visiting shelters, encampments, and other community locations Coordinate referrals to medical, behavioral health, and social service providers as appropriate Maintain accurate and timely case documentation in required systems Collaborate with internal teams and community partners to support client goals Required Skills and Abilities
Bachelor’s degree in social work, human services, or a related field, plus two years of professional social work or case management experience working with homeless and/or at-risk populations Ability and willingness to conduct field outreach, including visiting wooded areas or locations not intended for habitation to engage program participants Strong written and verbal communication skills Proficiency with Microsoft Office Suite, including Microsoft Excel Valid driver’s license, reliable transportation, good driving record, and personal auto insurance required Ability to drive a 15-passenger van Candidates must successfully pass required criminal background checks Bilingual (English/Spanish) is preferred. Physical Requirements
Prolonged periods of sitting at a desk and working on a computer. Must be able to lift 15 pounds at times. Must be able to attend occasional meetings at other FACETS office locations within Fairfax County, including 3 annual organization-wide fundraising events each year. Salary Range
$48,000 - $51,000 annually FACETS offers a competitive salary commensurate with qualification/experience. Benefits
