$10,000 Sign On Bonus available for eligible candidates (external only excluding rehires within 12 months, 12-month contract required) Directs daily operations of the care management department, including discharge planning, care coordination, progression of care, and social services. Ensures compliance with CMS CoPs for Discharge Planning as well as related federal and state regulations. Responsible to participate in Corporate Care Management meetings, cascade information, and implement strategies with standard work and processes. Develops and oversees the implementation and compliance with Care Management policies and standard operating procedures. Collaborates with interdisciplinary hospital leaders and executives to achieve organizational goals. Knowledge, Skills, and Abilities

Understanding of medical necessity for appropriate patient status and level of care [Required] Understanding of Care Management best practices [Required] Ability to research and reason best practices to support process improvement [Required] Strong verbal and electronic communication skills [Required] Ability to analyze, evaluate and monitor data in addition to provide action plans [Required] Education

Bachelor's [Required] Master's [Preferred] Field of Study

in Social Work or Nursing Work Experience

3+ of manager level (or above) experience in care management/social work leadership [Required] 5+ in care management leadership (manager level or above) in addition to 5 years of direct care management/utilization management experience and increasing leadership responsibility [Preferred] 5+ of care management experience performing hospital discharge planning [Required] Additional Information

If RN: Bachelor's of Science in Nursing [Required] OR If LCSW: Master's in Social Work [Required] OR Bachelor's in Social Work and Master's degree [Required] Licenses and Certifications
