Senior Change Realization Consultant

The Senior Change Realization Consultant is a senior contributor responsible for guiding strategic initiatives from launch to sustained realization. The role will lead change realization consulting engagements with major changes, design and implement targeted change interventions, and manage large-scale process and people transitions with proven methodologies. This role is responsible for educating leaders and colleagues on the skills and behaviors required to realize change, coach managers to reinforce realization, and provide data-driven recommendations to remove barriers and drive measurable outcomes. The role partners closely with program teams to develop comprehensive change realization plans, align sponsorship, and track effectiveness against business objectives. Job Duties: Execute change realization strategies aligned with mission and organizational goals, including impact assessments, stakeholder engagement, and communication plans. Assess the impact of change, identify key stakeholders, and develop solutions to address resistance and seven key dimensions of risk. Apply frameworks, establish change realization timelines for the change being requested, including communication planning and execution. Track and measure the adoption of changes to ensure success and make necessary adjustment. Collaborate, manage and continually improve the delivery, depth, and breadth of change throughout the Jefferson Enterprise. Research best practices and develops change leadership materials for organization-wide programs. Provide consultation and coaching with leaders and peers to support the development of critical competencies in the area of change leadership. Execute on the change realization strategy through alignment with the organizational strategy and the facilitation, design and delivery of education programs for Jefferson Leaders and colleagues. Support adoption of new technologies and workflows. Monitor staff feedback and adjust training as needed. Monitor KPIs for change realization success. Conduct qualitative and quantitative analysis related to change initiatives. Models Change Agent behaviors and habits to instill change culture throughout the organization. Build strong relationships and communications with key executive and senior-level business and clinical leaders to position process change work as a key enabler to strategy to execution efforts and champion belonging in change efforts. Model Jefferson values and habits in change efforts. Minimum Qualifications: Bachelor's Degree in related field. 4 years of experience leading and supporting teams through large, complex change initiatives. Demonstrated experience in providing change management support across a range of organizational functions and types of change including large scale transformational changes that impacts organizational culture and capacity. Demonstrated experience using internal or industry standard change management methodology like CAP, Prosci, SEF, etc. Strategic thinking and systems level leadership. Exceptional communication and stakeholder influence. Understanding of operations and environment. Ability to influence and lead change across diverse stakeholder groups. Highly collaborative and self-motivating and able to engage and motivate others. Keen attention to detail and advanced organizational and time management skills and ability to develop and manage a project plan incorporating change management principles. Proficiency in project management, process improvement, and change management tools. Preferred Qualifications: Master's Degree in Healthcare Administration, Education, or related field. Familiarity with change internal or industry standard change management methodology like CAP, Prosci, SEF, etc. Change Management certification preferred. Physical Demands: Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
