Physical Demand and Working Conditions
Valid drivers license & available car. Submit copy of current car insurance. Required to take all mandatory trainings. Adheres to HIPAA policies & procedures/compliance plan. Scheduled work hours include evenings and weekends.
Mental Health Peer Specialist - Qualifications and Competencies
High School Diploma preferred.
At least 3 years experience as a mental health recipient who has successfully utilized opportunities for personal growth.
Ability to respond appropriately to supervision and work as a team member.
Certified by the Academy of Peer Services or undergoing certification is preferred.
Schedule
Training Period: Mondays - Fridays 9am-5pm
Shift: Tuesdays - Saturdays or Sundays - Thursdays
Minimum one late shift a week required.
Location
Office: Baldwin, NY 11510
Traveling throughout Nassau County.
Duties and Responsibilities
Assists identified Supportive Housing and Apartment Treatment Program clients in community integration utilizing resources within the community.
Helps to increase socialization for identified clients when needed/requested.
Assists other housing agencies in applying for outside resources such as Wrap‑Around services from the Nassau County Office of Mental Health.
Provides supportive counseling and education in areas such as medication management, symptom management and developing coping strategies.
Development of WRAP (Wellness Recovery Action Plan) plans with clients.
Provides post‑discharge follow‑up.
Maintain contact with treatment team, Health Home care coordinator, Housing staff to help maintain coordination of services.
Participates in staff development activities.
Participates in routine supervision with the team leader.
Responds to emergency situations as appropriate.
Submits all required documentation and reports in a timely manner.
Accepts and follows through on all other duties as assigned.
Attends work‑related workshops/conferences as program schedule permits.
