Base Salary: $75,000 – $90,000+ (based on experience & book of business)

Role – What You’ll Do

Build and maintain strong referral relationships within senior living communities

Drive census growth by meeting/exceeding sales goals

Identify resident care needs and coordinate appropriate services

Act as a resident advocate to ensure smooth transitions to home care

Manage the referral-to-admission process efficiently

Collaborate with physicians, nurses, and interdisciplinary teams

Provide education to residents, caregivers, and community partners

Maintain consistent communication with executive leadership and referral sources

Qualifications

Active book of business within the territory (REQUIRED)

Healthcare sales experience (home health, SNF, DME, hospice, or clinical liaison) preferred

Clinical background (RN, LPN, MSW, etc.) preferred

Strong communication, presentation, and relationship‑building skills

Detail‑oriented with strong organizational skills

Ability to travel within the territory

Valid driver’s license, auto insurance, and reliable transportation (required)
