We are seeking a detail-oriented, customer service-focused Intake Specialist to join our growing team. The ideal candidate is passionate about people and the world of behind-the-scenes health care.
Responsibilities
Coordinate the screening and intake of Home Care, Supportive Home Care, and Hospice referrals. Assist the Intake team by communicating with referral sources, coordinating referral information and managing workflow in the department.
Enter patient information into the electronic health record accurately with appropriate medical terminology and abbreviations.
Oversee incoming referrals through fax, phone, or email and obtain information needed to accept referral.
Serve as the first/primary contact for phone calls within the department. Manage multiple lines and relay messages/information to appropriate team members and departments.

Qualifications
High School Diploma or equivalent
2-3 years of office experience
Familiar with MS Office and Medical Terminology
Superb communication, organization, prioritization, and customer relations skills
Current/Valid WI Driver’s License and reliable/insured vehicle
Shift: Every other weekend – Saturday & Sunday 8am-4:30pm + holiday rotation.
Hourly, Non-Exempt. Department: Intake.
