Supportive Housing Specialist EMPLOYMENT TYPE:

1391 Responsibilities
Provide targeted case management such as establishing rapport with clients, writing comprehensive treatment plans, providing counseling and support, direct skill teaching, advocacy and assistance in obtaining/maintaining benefits, budgeting, treatment, medical services, employment, transportation, etc. Maintain family and natural supports as well as other collateral contacts to ensure quality client-centered care, including assessing and involving natural supports in the treatment process, networking with homeless service providers, liaison with landlords, and overall case coordination. Assist in understanding federal/state guidelines for rental subsidies, tenant rights and responsibilities, and basic housing law; conduct outcome surveys as required. Maintain documentation in compliance with agency and program standards. Participate in staff meetings, supervision, trainings and other meetings as assigned. Additional duties as required. Education:
Bachelor’s Degree in human services or equivalent education and experience Experience:
2 years experience working with the dually diagnosed (mentally ill, substance abuse) Licensure/Certification/Registration:
Valid driver’s license. CPR/First-Aid Certification within 6 months of hire desired. Benefits

Compensation:
$19.28/hr minimum, and up commensurate upon experience. Actual rates are determined at the time of offer based on relevant experience and adherence to internal equity. Part-time pro-ration applies where applicable.
