
Bilingual (English/Spanish) Community Health Worker - Remote in Texas

Full‑time position (40 hours/week, Monday – Friday). Employees are required to have flexibility to work any of our 8‑hour shift schedules during our normal business hours of 8 am – 5 pm CST.
2 weeks of paid training. The hours during training will be 8 am to 5 pm CST, Monday – Friday. Training will be conducted virtually from your home.
Location: State of Texas. If you reside in the state of Texas, you will enjoy the flexibility to telecommute as you take on some tough challenges.
Primary Responsibilities
Documents all members interactions in a concise manner that is compliant with documentation requirements established in Job Aids
Conducts out‑bound calls through Autodialer for program requirements including, but not limited to members appointment scheduling, provider outreach, assess for special needs, Texas Health Steps and access to care for farmworker children
Schedules and coordinates for members transportation, and follow‑up for physician appointments as needed
Provides clerical and / or administrative support to clinical staff and managers for special projects and reports
Provides excellent customer service by serving as a resource to all internal and external customers
Maintains knowledge of all health plan benefits, network, regulations, HHSC requirements
Informs members of the available community resources available to help meet their needs, including but not limited to: TANF, SNAP, CSHCN, WIC
Complete appropriate assessments and care plans as needed to streamline members' access to care
Submits internal and external referrals
Assist all members with barriers to care
Performs all other related duties as assigned
Meet and maintain metrics daily

Required Qualifications
High School Diploma/GED (or higher) OR 4+ years of medical customer service experience administration
2+ years of customer service experience
2+ years of experience in medical customer service
Intermediate level of computer proficiency with Microsoft Outlook, Teams, Excel, Word and the ability to navigate through multiple web applications
Reside in a location that can receive a UnitedHealth Group approved high‑speed internet connection or leverage an existing high‑speed internet service
Access to a designated quiet workspace in your home (separated from non‑workspace areas) with the ability to secure Protected Health Information (PHI)
Reside in the state of Texas
Bilingual - Spanish and English
Preferred Qualifications
Experience working within the health care industry and with health insurance
Experience working in a hospital, physician's office or medical clinic setting
CHW, CNA, or MA experience
Ability to do frequent speaking, listening using a headset, sitting for long periods, use of hands/fingers across keyboard or mouse
