Job Description We are seeking a motivated Regional Health and Wellness Director to join our growing team. This position will require travel for coverage in Connecticut and Massachusetts. The focus for this position will include oversight of our teams while ensuring compliance with state and federal regulatory guidelines as well as monitoring and updating company policies and procedures.
Salary Range: $140,000 - $150,000 Yearly
Responsible for all areas of the Health & Wellness department (compliance, planning, assessments, directing, budget, staffing, hands‑on care) in accordance with current state regulations and company policies and procedures
Ensure thorough resident assessments are being conducted
Ensure resident care needs are met and maintained according to physician orders, state regulation, and company policies and procedures
Ensure the effectiveness of the resident’s care plan is documented through written progress notes and through verbal communication with residents and associates
Evaluates the health and wellness Directors’ skill competencies and overall performance to ensure compliance to state regulations and company policies and procedures
Implement training programs and in–services for all Health and Wellness Directors in accordance with state regulations
Provide ongoing leadership to the Health & Wellness Directors and ensure staffing levels and shift schedules are operating at the highest level
Hiring, training, and mentoring Health and Wellness Directors
Develops and maintains department budgetary guidelines for staffing and supplies
Develop, implement, and maintain compliance with assisted living and memory care, to ensure safety, and clinical standards.
Serve as a member of the Regional Leadership Team responsible for the overall strategic direction and management of clinical strategies in assigned portfolio
Monitor and analyze resident care data in accordance with state regulations and company policies and procedures
Perform audits to ensure thorough resident assessments, care plans, and documentation in EHR and EMAR. Assist with action plans and plans of correction
Ensure compliance with resident rights and work to resolve resident grievances
Schedule site visits to provide supervision, mentorship, and clinical expertise. Communicate and monitor the overall operation and outcomes of the nursing department
Assist with implementing best practices
Implement orientation training programs and in‑services for all nursing teams in accordance with state regulations and company policies
Train, coach and counsel nursing staff
Participate in prospect/admission process and discharges, as needed
Develop, implement, and lead the company’s quality assurance program and develop benchmarks and monitoring measures to ensure compliance
Develop and maintain department budgetary guidelines for staffing and supplies
Attend care conferences as needed
Work with third‑party vendors/partners to improve outcomes/relationships
Participate in mock survey visits and attend annual/complaint survey visits, as needed
Audit level of care assessments for accuracy and compliance
Participate in recruitment efforts, including but not limited to interviews, and selection process
Orchestrate onboarding for community Directors of Health and Wellness
Work with Executive Director and community to ensure appropriate coverage during vacancies
Communicate appropriately to VP of Health & Wellness/Operations, on areas of significant concerns, risk and sentinel events within the designated portfolio
Participate in corporate meetings/calls (nursing, sales, operations) as needed
Attend home office and off‑site meetings, as needed
Qualifications
Graduation from an accredited School of Nursing, with a current RN license in CT and MA
At least 2 years’ experience with long‑term care, assisted living, home health or hospital setting preferred
Experience working with residents with Alzheimer’s or other related dementias
Ability to handle multiple tasks
Knowledge of federal and state regulations, and of nursing practices, techniques and methods applied to health and wellness resident
Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) with the ability to learn new applications
While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell
An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics
