Job Summary and Qualifications The Facility Case Management Director oversees and coordinates all department activities, ensuring compliance with policies and regulatory standards while integrating patient care and discharge planning across departments. Responsible for continuity of care, optimal resource utilization, and achieving outcomes through interdisciplinary collaboration and stakeholder engagement. Acts as a subject matter expert with strong leadership, communication, and change management skills, while managing multiple priorities with minimal supervision. Responsibilities Directs and evaluates departmental operations including case management models, staffing, onboarding, competencies, and use of information technology. Allocates resources effectively to meet productivity and quality goals. Oversee core case management functions and ensure adherence to care coordination and discharge planning processes. Collaborate with Facility CFO and Division CM Leader to meet organizational goals. Monitors performance, analyzes data, and implements sustainable improvements. Ensures compliance with policies, SOPs, and maintains strong relationships with key stakeholders (CMO, CNO, Legal, etc.). Upholds Code of Conduct and Mission & Value Statement; performs additional duties as assigned. Qualifications Bachelor’s degree required; master’s in nursing, Health Administration, or Business Administration preferred. Required licensure: RN, LCSW, or LMSW. Minimum 3+ years in acute care hospital case management and 2+ years in leadership roles.
