Program Director (Licensed Social Worker) Vantage at Wakefield

Overview

Vantage at Wakefield is currently looking for a licensed Social Worker to take on the newly created Program Director position at our facility. Prior experienced working in behavioral health settings are preferred.

Responsibilities

• Participate in development of an individualized care plan with the Social Service Director, Recreation Director, and nurse management team
• Identify and manage barriers to achievement of care plan goals
• Identify and implement effective interventions based on opportunities identified in members plan of care
• Work collaboratively with the Social Service Director, Recreation Director, and nurse management team

Qualifications

Licensed Social Worker. Prior experience in behavioral health settings preferred.
