Stonecreek Health and Rehabilitation is a 120‑bed skilled nursing facility. Stonecreek Health and Rehabilitation is currently seeking a Full‑Time Activity Director to join our team of dedicated health care professionals.

In this role, the Activities Director will oversee the planning, organizing, developing, and directing the overall operation of the Activity Department to promote the physical, mental, and psychosocial well‑being of residents.

Activity Director Certification

This position requires creativity, compassion, and a desire to care for the residents in an optimistic and engaging manner.

Responsibilities

  • Plan, organize, and develop an engaging monthly activity calendar to meet the needs and interests of each resident.
  • Provide a positive and creative environment for residents to enjoy a multitude of activity options.
  • Perform necessary assessments as it pertains to each individual resident.
  • Develop a plan of care based on assessment, implementing activities in accordance with the comprehensive assessment and the interests and physical, mental, and psychosocial well‑being of each resident.
  • Serve as the direct supervisor of the Activity Assistant(s).
  • Work closely with the Marketing Director in planning, organizing, and preparing special events/holiday celebrations.
