Role Overview Responsibilities and working hours: Monday - Friday, 8 a.m. to 4:30 p.m. The Inpatient Social Worker carries a patient caseload at our 101-bed Psychiatric Hospital in Brooklyn Park, Minnesota, serving ages primarily 5-35. This role is part of a four-year SAMHSA Grant funded position to primarily serve Native American patients.

The role focuses on facilitating groups, treatment planning, discharge planning, care coordination, communication with caretakers, community providers, and others involved in patient care. It also involves community outreach to the Native American community and providing social work services across the continuum of care, including post-discharge community support.

Key Responsibilities

Uphold and apply clinical knowledge and principles of emotional and behavioral health practices, including Indigenous Health Disparities.

Assess, plan, implement, and evaluate psychosocial services for patients; share expertise with the treatment team throughout the care continuum.

Develop and revise treatment and complex discharge plans; communicate with patients, caretakers, tribal entities, treatment team, and direct care staff.

Provide psychoeducation regarding diagnosis, symptom management, recommendations, discharge planning (including leading discharge meetings), safety plans, patient rights, financial impact issues, Indigenous Health Disparities, and other relevant patient care matters.

Coordinate care with external providers, agencies, tribal entities, government and private payer networks; facilitate transition to appropriate levels of care following hospitalization.

Lead child, adolescent, and young adult therapy/process groups on inpatient units.

Complete Think: Kids: Collaborative Problem Solving (CPS) training and use the CPS Assessment and Planning tool to help children develop skills for challenging behaviors; offer culturally appropriate, trauma-informed support in partnership with the Spiritual Cultural Support Liaison.

Serve as communicator and liaison within multidisciplinary teams; understand cultural values, beliefs, and practices of Native American groups.

Navigate complex health and social systems, including provisions of ICWA.

Maintain awareness and personal care to address secondary traumatic stress related to lived experience and work experience within the Native American community and mental health systems.

Qualifications

Master's degree in Social Work with licensure in the State of Minnesota (LGSW required, LICSW preferred).

Two years of experience in a mental health care setting (preferred).

Knowledge of Native American communities (urban and reservations) and understanding of tribal/intertribal differences.

Experience working with Native American children, adolescents, and families (strongly preferred).

Credible role model in the Native American community with established relationships with key partners, including community agencies and tribal entities.

Experience using electronic health records.

Proficiency with Microsoft Office products (Word, Outlook, Teams).

Strong written and verbal communication skills.

Ability to coordinate care with relevant staff.

Capability to shift between dissimilar tasks smoothly and rapidly.

Strong decision-making skills in stressful situations and circumstances.

Compensation and Benefits Compensation ranges from

$30.00 to $38.60 per hour

depending on licensure and experience. The pay rate is influenced by skills, experience, education, and market conditions, while internal equity is also considered.
