Sequoia Care works in long‑term care and rehabilitation settings, including outpatient and home health. We deliver exceptional Physical, Occupational, and Speech Therapy services focused on quality outcomes for each individual.
Summary:
Demonstrates accountability for program development, quality improvement, staffing models, and operational optimization in an interdisciplinary environment in outpatient and/or home health settings. Adheres to compliance with state licensure, payer requirements, company policies, professional practices, and ethical standards.
Responsibilities:
Recruit, screen, select, hire, train, coach, counsel, and develop a team of qualified and highly motivated individuals; assists with student programs.
Effectively communicate with therapy and community teams to direct optimal resident care and deliver direct therapy as appropriate.
Participate in and direct screening activities and prepare resident documents in a timely manner for skilled intervention when needed.
Manage personal and team documentation for completion and timeliness compliance.
Proactively schedule residents and secure staff to meet treatment requirements.
Conserve resources by controlling cost, adhering to budgets, and using prudent judgment related to expenses incurred by the company.
Maintain individual and team efficiency and productivity.
Support and demonstrate a professional image and attitude with clients, families, colleagues, customers, and care partners.
Support departmental goals and overall company objectives.
Manage adherence to therapist credentialing with payer sources.
Manage clinic adherence to accreditation standards and assist with surveys as required.
Comply with all state and federal laws, regulations, and standards of ethical conduct.
What We’re Looking For:
Bachelor’s degree in physical or occupational therapy from a four‑year college or university, or equivalent combination of education and experience.
Applicable, valid, and unencumbered state professional license as a PT, OT, SLP, PTA, or COTA.
Current NPI and credentialing status with applicable payer groups.
Current CPR certification.
