DUTIES
Independently manage and coordinate the development and implementation of client‑based habilitative programming (e.g., individual program plans, behavior guidelines) for approximately 16–24 individuals with developmental disabilities at the Cambridge Developmental Center.
Chair the interdisciplinary team and coordinate services from all internal and external support services (speech, medical, pharmacy, social services, psychology, psychiatry, vocational, education, recreational, occupational therapy, physical therapy, dental, dietary) to meet client needs and ensure quality care.
Ensure all services are therapeutic, compliant with rules, and effective to promote habilitation; observe program implementation and recommend changes.
Formulate and coordinate annual comprehensive functional assessments for each client, gathering information from all necessary disciplines.
Complete comprehensive annual individual plans and monthly analyses of each client’s status, needs, trends, and recommendations.
Call and chair special team meetings as required.
Coordinate input from all services into individual program plans and resolve discrepancies or conflicts.
Develop client training goals based on team recommendations; ensure documentation meets Medicaid and licensure standards, correcting deficiencies as needed.
Supervise assigned staff: hire, interview, train, orient, evaluate, and manage leave forms.
Participate in hearings and disciplinary proceedings; author consents for behavior implementation and collaborate with health and safety to develop prevention plans.
Conduct routine rounds of residential and service facilities to ensure active treatment and proper documentation; prepare for and collect data for team meetings.
Work with program disciplines to establish individual goals, objectives, and priorities; write and modify programs as needed, ensuring compliance.
Provide information to families, guardians, community agencies, and county boards of developmental disabilities; support survey teams and write compliance plans.
Participate in planning meetings, community outreach activities, workshops, conferences, mandatory in‑services, committees, and perform other duties as required.
QUALIFICATIONS
Minimum one year of experience working directly with persons with intellectual disabilities or other developmental disabilities.
Must hold at least a bachelor’s degree in a professional human services category (e.g., Psychology, Education, Anthropology) or be a registered nurse, doctor of medicine, or osteopath.
Supplemental: Experience with Intermediate Care Facility and/or County Board is preferred.
Employment contingent upon successful completion of a criminal background check per OH R.O.C. Section 5123.081.

May be exposed to clients who display unpredictable or aggressive behavior.
Required to work a flexible schedule.
May be expected to perform temporary duties away from the developmental center location.
