Centinela Hospital Medical Center is seeking a
Director of Case Management
to lead a high-performing team and drive exceptional patient care, outcomes, and operational efficiency.
What You’ll Do
Lead and develop a team of RNs, Social Workers, and Case Management staff
Oversee
utilization management, discharge planning, and care coordination
Ensure patients receive timely, appropriate, and cost-effective care
Drive initiatives to improve
patient outcomes and reduce readmissions
Use data and quality improvement tools to optimize performance
What We’re Looking For
Active
California RN license
(required)
BSN required
(MSN or advanced degree a plus)
OR LCSW with 5+ years post-grad experience (if applicable)
5+ years in Case Management
Strong analytical skills and experience with utilization review
Knowledge of
InterQual / Milliman
criteria preferred
