Job Description The Independent Living Program Coordinator will provide group services, oversee individual services and provide individual services as requested to clients who have an independent living plan to strengthen their abilities and life skills to live independently while preserving family and community bonds.

Coordinates and conduct monthly Regional Youth Advisory Council meetings.

Maintains communications and working relationship with Kansas Youth Advisory Council members regarding current Independent Living activities in the State.

Attends quarterly DCF Independent Living meetings as requested by Program Manager

Maintains contact with DCF Independent Living Coordinators

Assist in providing information and coordinating attendance for participants to attend IL computer camps, conferences, events, and meetings.

Coordinates and conduct tours to post-secondary education sites

Obtains and distributes resource and scholarship information.

Maintains working relationship with Kansas Kids @ GEAR UP representative.

Serves as resource for workers regarding specialty areas for youth exiting the child welfare system.

Provides direct services to youth exiting the child welfare system.

Staff cases and assists case team with implantation of IL case plan tasks.

Completes worker child visits for OPPLA.

Completes Casey Assessments, Transition Plans, Permanency Pacts, Application for Aged Out Medical Program and Exit Interviews for OPPLA and referred youth.

Transport clients as needed.

Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres all pertinent Saint Francis Ministries Standard Operating Procedures (SOP’s), rules, personnel policies, and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and terms of DCF contract.

Reports unusual incidents through appropriate Risk Management, clinical and safety channels.

Ensures clients’ rights are protected.

Is knowledgeable of and follows all safety procedures.

Requirements

Bachelor’s degree in Human Service field from an accredited College or University preferred

Prefer two years’ experience in the children and family services field.

Must be 21 years of age.

Must pass a drug screen, MVR, KBI, Child Abuse and Neglect Central Registry Clearance check and an Adult Registry Check

FBI Fingerprint check

Lifting requirement of 50 lbs.

Must have a valid driver’s license, acceptable driving record and reliable transportation
